Disabilities and Other Supports Form

Our Next Gen ministries are committed to ministering to every child. Completing this form will provide us with all of the necessary information to make your child's time in our ministries extraordinary. We want to be the least restrictive environment for your child, and to do this we have support volunteers who oversee each area and can offer support to many children who may not need a 1:1 at all times. Children/Students will always be encouraged to attend their age-appropriate worship experience as well as their small groups. Inclusion is always our goal, when appropriate. However, by fully answering the questions in the form below we get to learn more about your child/student and adapt to their needs and determine if a 1:1 Buddy is required to have a safe and fun experience in RC Kids or Tribes Jr. High. Additionally, during the 10:15am and 12pm service, our Sensory Room is available for Preschool-6th Grade. This is a sensory friendly atmosphere with skilled volunteers and an adapted curriculum provided. This space is the last room on the right in the Next Gen hallway. All kids who need additional support are encouraged to be dropped off in this space, volunteers will assist them to their groups after check-in. Completing this form does not guarantee your child will receive a 1:1 buddy. This form is for any child who may need additional support, accommodations, or adaptations on a Sunday morning or a weekday ministry service.
